This application is designed to prepare the individual for the Pennsylvania Certification Board’s (PCB) credential of Certified Recovery Specialist. It is important to understand that by obtaining this credential, you are disclosing to your community, co-workers, and potential clients that you are in long term recovery from drug and alcohol addiction. There are no wrong answers so please complete the following questions honestly and thoughtfully. Applicants must:

Complete COCA’s CRS Training Application form • Commit to attending all courses and exam • Have been in recovery for at least 18 months • Be able to provide evidence of a High School diploma, GED, or an official transcript from a college or university
